Insurance Coverage |
In order for treatment to be a covered benefit (Covered means your insurance company will pay claims after you have satisfied your policy’s individual and family deductible and co-pays.) by your insurance company‚ it must be considered medically necessary. Most insurance companies‚ including Medicare, consider treatment of symptomatic varicose vein disease to be medically necessary when you have met the following three criteria.
- You must have symptoms related to vein disease such as pain or aching in your legs‚ swelling‚ bleeding from a varicosity‚ phlebitis or ulcer.
- You must have documented evidence of reflux in one of the main superficial vein or veins by ultrasound.
- You often must complete a course of conservative management to include wearing graduated compression hose, elevating your legs when possible and taking non-steroidal anti-inflammatory medication such as Tylenol or Motrin for a period of time. (The required time is different for each insurance company.)

Next Steps |
When you have met the criteria above‚ we’ll send a request to your insurance company for a pre-determination of benefits. The request contains all the information about the symptoms you are having‚ the findings on ultrasound and a statement that you have undergone conservative management. With the request for pre-determination of benefits‚ we ask your insurance company to approve all the treatments Dr. Robelen believes will be necessary to address your disease. Your insurance company will review your individual policy and give us a response. Most insurance companies will allow us to perform the full range of treatments necessary to take care of your disease.
Once we receive your pre-determination of benefits we will go over it with you and can then schedule your procedures—usually over several sessions. At that time we will be happy to estimate your out of pocket expense for the entire treatment. This is only an estimate, as the number of treatments you require will be determined by your response to treatment. to allow us to track your treatment progress. |
