Important Info for Your First Visit: |
- Please arrive 15 minutes early to allow time to complete any additional paperwork.
- Plan on your initial appointment lasting approximately 90 minutes.
- Please do not bring children to your appointment unless you have an adult caregiver
who will remain with them in the lobby.
- You MUST confirm your appointment by noon the day before by calling (618) 998-8346 for
patients of our Marion, IL clinic or (270) 444-7722 for patients of our Paducah, KY clinic.

Medical History and Information |
| The initial visit allows our patients and staff to get to know one another. You will be asked to complete a comprehensive medical history with a focus on your venous history. Forms can be printed and completed prior to your appointment. (See links to forms above.) |

Physical Exam |
| We will give you a complete physical exam, which may include a screening ultrasound to diagnose your specific varicose disease. The precise diagnosis will tell us which treatment options may be most effective for your individual case. Photographs of your legs may be taken. |

Discussion of Treatment Options |
| After the exam, we will discuss your best treatment options. Some treatments require more than one session to achieve desired results and we will talk about this possibility. We will also go over procedures in detail so you know exactly what to expect from subsequent visits. It is important for you to feel comfortable with the prescribed treatment, so feel free to ask any questions that come to mind. |

Things to Bring to Your First Appointment |
To make your first session as productive as possible, please bring the following items with you:
- The name and phone number of your primary physician
- Your insurance card
- Any forms completed prior to the initial consultation
For the convenience of our patients, we provide PDF versions of our required patient forms. You can print our New Patient Forms at home and bring completed copies to your first visit.
Before your next appointment we will work with your insurance company to determine coverage. If there is an out-of-pocket expense, we can, at your request, provide that information and payment options to you prior to the treatment.
Please note that we WILL be able to see you even if you don't bring all of the above items to your first visit. |
